NEEDS ASSESSMENT & INDIVIDUAL SERVICE PLANNING

Interviewing a person’s past and the individuals who represent their core support group can reveal preferences, gifts, strengths, and talents. Assessing needs and service planning is a process and ongoing dialogue with individuals you support and their care teams. In this course, you will learn that each resident has unique capabilities and care needs.

AUDIENCE: Assisted Living Staff, House Managers, Administrators

OBJECTIVES: 

• Identify the necessity of a needs assessment
• Distiguish the elements involved in individual service plans
• Understand changes in condition, including the screening and planning needs required
• Consider the evaluation and review process for needs assessment and ISPs

COURSE FORMAT: This course is a recorded, asynchronous, self-paced, online interactive training, with no access time limit or due date to complete it once enrolled.  In addition to the record presentation, additional resources and handouts are made available inside the course.
