NEEDS ASSESSMENT & INDIVIDUAL SERVICE PLANNING
Interviewing a person’s past and the individuals who represent their core support group can reveal preferences, gifts, strengths, and talents. Assessing needs and service planning is a process and ongoing dialogue with individuals you support and their care teams. In this course, you will learn that each resident has unique capabilities and care needs.

OBJECTIVES:
- Identify the necessity of a needs assessment
- Distiguish the elements involved in individual service plans
- Understand changes in condition, including the screening and planning needs required
- Consider the evaluation and review process for needs assessment and ISPs
